Free Imaging records are now available through Clearpath! Instead of picking up a CD, you can access your records at any time, from anywhere.

POM MRI & Radiology Centers' patients can now easily access their medical imaging and reports via Clearpath's website.

How to Get Clearpath

If you have recently been seen at POM MRI, you will receive an email when your records are ready, including your Clearpath login information.

1. Simply create an account at CLEARPATH.COM.

2. You will be asked to enter your First and Last Name, Date of Birth, Email and Time Zone.

3. Once you have created an account you will get a temporary password sent to your email.

4. Use the temporary password and your email address to log in to Clearpath.

5. Once logged in request your records.

How to Request Your Records

1. To request your records, select the "Request Records" tab.

2. To request a record, select the Request Record button on the bottom right corner.

3. Select "Record Custodian" (Provider Location).

4. Select the records to request, Body Part, and Date Range.

5. Agree to the terms and conditions and submit.

*Prior to requesting your records, please ensure a copy of your Photo ID is attached to your profile under the "My Identity" tab.

Once your request has been submitted, the facility will review your request.
Once the request has been processed and approved, you will see your images under the "My Records" tab.
